Insurance Information

Dr. Buch is an approved provider for most insurance plans including Medicare, Secure Horizons, Aetna, Blue Cross/Blue Shield, United Healthcare and Cigna. Please check with your insurance carrier prior to your appointment or call us if you have questions about your insurance approval.

Appointments & Emergencies

Appointments are scheduled by calling 972-566-4400. If the scheduled appointment cannot be kept, please call our office in advance to reschedule. Repeated missed appointments may result in discharge from the practice. If you are experiencing a medical emergency, call 911. If you are experiencing a problem between appointments, please call and we will decide together if the problem can be handled by phone or visit. We respectfully ask that you schedule separate appointments for each injury or illness.

Fees & Payments

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa, and Discover for your convenience. If we are a participating provider of your insurance company, we'll bill them. However, payment is the patient's responsibility. We'll help in any way we can to assist you in handling claims.

Prescriptions & Refills

All medications, including refills, are prescribed based on your current condition. Follow-up appointments are scheduled to monitor your condition. If the last appointment was not kept, refills may not be allowed.

Calls for prescription refills should be placed to your pharmacy. The pharmacy will contact our office. Your medical record, which is only available in the office, is needed to determine whether a refill should be issued. Therefore, it is your responsibility to call the pharmacy for refills 24 hours prior to running out of the medication. We do not fax new prescriptions. Replacement prescriptions for prescriptions that were lost will be reissued at a charge of $15.00 per prescription. Pain medications will not be refilled on weekends.

Request for Medical Files

We are happy to provide files for insurance, job, or other medical-related purposes. We will only release these files to you or your designee. Due to confidentiality, we only accept requests from recognized insurers or yourself. If you wish to obtain copies of your files, you must visit our offices in person and sign a file release form.
