The Dallas Limb Restoration Center is a national referral center that specializes in treating a wide array of musculoskeletal conditions. From simple orthopedic ailgments such as carpal tunnel syndrome to complex problems like a failed total joint replacement or bone tumor that requires major reconstuction we provide thorough evaluation, treatment, and compassionate care. Dr. Buch brings over 25 years experince treating the most complex orthopedic cases and has performed over 14,500 joint procedures.

Appointments & Emergencies

Appointments are scheduled by calling our office at (469) 443-0924. If the scheduled appointment cannot be kept, please call our office in advance to reschedule. If you are experiencing a medical emergency, please call 911. If you are experiencing a problem between appointments, please call and we will decide together if the problem can be handled by phone or visit.

Fees & Payments

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa, and Discover for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility and we will help in any way to assist you in handling claims.

Prescriptions & Renewals

All medications, including refills, are prescribed based on your current condition. Follow-up appointments are scheduled to monitor your condition. If the last appointment was not kept, refills may not be allowed.

Calls for prescription refills should be placed to your pharmacy. The pharmacy will contact our office. Your medical record, which is only available in the office, is needed to determine whether a refill should be issued. Therefore, it is your responsibility to call the pharmacy for refills 24 hours prior to running out of the medication. We do not fax new prescriptions. Pain medications will not be refilled on weekends.

Request for Medical Files

We are happy to provide files for insurance, job or other medical related purposes. We will only release these files to you or your designee. Due to confidentiality, we only accept requests from recognized insurers or yourself. If you wish to obtain copies of your files you must visit our office in person to sign and file a release form.
